A superbill is a detailed receipt that you can submit to your insurance company for possible reimbursement. We collect payment at the time of service and provide all necessary documentation for you to file an out-of-network claim. Reimbursement depends on your individual insurance benefits.

A superbill is a detailed receipt that patients can submit to their insurance company for possible reimbursement after paying out-of-pocket for physical therapy.
Since we are out-of-network with insurance plans, we collect payment at the time of service and provide a superbill upon request.
What Is Included on a Superbill?
A superbill typically includes:
Patient name and date of birth
Provider name and credentials
Practice information (NPI, EIN, address)
Date(s) of service
Diagnosis codes (ICD-10)
Treatment codes (CPT codes)
Amount paid
It contains everything insurance needs to determine reimbursement eligibility.
How the Process Works (Step-by-Step)
1️⃣ Patient Pays at Time of Visit
Payment is made directly to the clinic.
2️⃣ We Provide the Superbill
After the session (or at the end of a care plan), we generate a superbill.
3️⃣ Patient Submits to Insurance
The patient submits the superbill to their insurance company:
Through an online portal
By email
Or by mailing it in
4️⃣ Insurance Reviews the Claim
The insurance company processes it as an out-of-network claim.
5️⃣ Patient May Receive Reimbursement
If they have out-of-network benefits, insurance may reimburse:
A percentage of the visit cost (commonly 50–80%)
After the deductible is met
Reimbursement is sent directly to the patient — not to the clinic.
Important Things Patients Should Know
Reimbursement depends entirely on their specific plan.
Some plans have no out-of-network benefits.
Deductibles must often be met first.
We cannot guarantee reimbursement.
We always recommend patients call their insurance and ask:
Do I have out-of-network physical therapy benefits?
What percentage is reimbursed?
Is there a deductible?
Is there a visit limit?
